What Is Mounjaro?

Mounjaro is the brand name for tirzepatide, a once-weekly injectable dual GIP and GLP-1 receptor agonist made by Eli Lilly and FDA-approved for type 2 diabetes. It is the same molecule as Zepbound, which is FDA-approved specifically for chronic weight management and obstructive sleep apnea. Tirzepatide is currently the most effective approved medication for both glycemic control and weight loss.
How Mounjaro differs from semaglutide (Ozempic / Wegovy)
Mounjaro is a 'twincretin' — it activates both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors. GLP-1 reduces appetite, slows gastric emptying, and improves insulin response. GIP appears to enhance insulin secretion and may improve adipose tissue lipid handling. The dual mechanism produces greater HbA1c reductions and substantially greater weight loss than semaglutide in head-to-head trials (SURPASS-2, SURMOUNT-1).
Approved uses, dosing, and results
Mounjaro is FDA-approved for adults with type 2 diabetes as an adjunct to diet and exercise. Doses are 2.5 mg weekly (starting, not therapeutic), titrated through 5, 7.5, 10, 12.5, and up to 15 mg weekly. In the SURPASS trials, tirzepatide lowered HbA1c by 1.9-2.6% and produced average weight loss of 15-22% — significantly more than semaglutide at the highest doses. Cardiovascular outcomes trials are ongoing.
Side effects and considerations
Side effects mirror other GLP-1s: nausea, vomiting, diarrhea, constipation, and abdominal pain, mostly mild and dose-titration dependent. Serious but rare risks include pancreatitis, gallbladder disease, and the thyroid C-cell tumor boxed warning shared with the GLP-1 class — contraindicated in personal or family history of medullary thyroid carcinoma or MEN2. As with all GLP-1/GIP medications, preserving lean mass through high protein intake and resistance training is essential.

Blood sugar improvements appear within the first weeks. Meaningful weight loss typically begins by 8-12 weeks and continues through dose escalation. Peak weight loss is reached around 72-88 weeks.
