What Is Blepharoplasty?
Last reviewed: May 2026 · Haute MD Editorial Team
Blepharoplasty (eyelid surgery) is a procedure that removes excess skin, muscle, and fat from the upper and/or lower eyelids to improve hooding, puffiness, and the rested or refreshed appearance of the eyes. Upper blepharoplasty addresses drooping upper eyelid skin that creates a tired or hooded appearance and can impair vision in severe cases. Lower blepharoplasty addresses under-eye bags, puffiness, and hollowing.
Upper blepharoplasty
Excess upper eyelid skin (dermatochalasis) is removed through an incision placed in the natural eyelid crease, making the scar nearly invisible when healed. In cases of significant ptosis (drooping), the levator muscle may also be tightened. When upper eyelid hooding significantly impairs the visual field, upper blepharoplasty may be covered by insurance.
Lower blepharoplasty
Lower blepharoplasty can be performed through an external incision just below the lower lash line (transcutaneous) or through the inside of the eyelid (transconjunctival) — the transconjunctival approach leaves no visible scar. Fat may be removed, redistributed, or both. Lower blepharoplasty is often combined with fat grafting or filler to address hollowing under the eyes.
Recovery from blepharoplasty
Initial healing takes 7-14 days, with bruising and swelling that can be covered by glasses. Most patients are comfortable in public at 2 weeks. Residual swelling can persist for 2-3 months. Final results are visible at 3-6 months.
Frequently Asked Questions
Is blepharoplasty covered by insurance?
Upper blepharoplasty is covered by some insurance plans when excess upper eyelid skin significantly impairs the visual field. A visual field test performed by an ophthalmologist is required to document functional impairment. Lower blepharoplasty is considered cosmetic and is not typically covered.
Can blepharoplasty make you look younger?
Yes. Blepharoplasty is one of the most effective procedures for facial rejuvenation — the eyes are the first area people look at, and improved upper and lower eyelid appearance can make patients look 5-10 years younger and more rested.
What is the difference between blepharoplasty and a brow lift?
Blepharoplasty removes excess eyelid skin. A brow lift elevates the brow position, which reduces the amount of skin that descends onto the upper eyelid. Both may be needed if the brow position is low as well as if there is true upper eyelid excess.
What age is appropriate for blepharoplasty?
There is no specific age requirement. Most patients seeking blepharoplasty are in their 40s-60s, but younger patients with hereditary under-eye bags or significant upper eyelid hooding can benefit earlier.
