The experts at PRMA Plastic Surgery answers all your questions on breast explant surgery or breast implant removal.

What is Explant Surgery?

Explant surgery is an option for patients who experience chronic complications associated with their breast implants, have concerns about developing implant-associated cancer, or who simply no longer wish to have implants.

How is the procedure performed?

Explant surgery should be performed by a board-certified plastic surgeon. During the procedure, the surgeon removes the breast implant and all of the surrounding scar tissue (referred to as a capsule). The procedure is typically performed in an outpatient setting.

Who is a candidate?

Any patient who is experiencing complications associate with their breast implants is a candidate for explant surgery regardless of whether their implants were placed for cosmetic purposes or for breast reconstruction following breast cancer surgery. Typical reasons patients choose to have their breast implants removed include:
• Capsular contracture
• Visible implant rippling (the appearance of "waves" or "creases" under the skin)
• Implant rupture
• Asymmetry
• Breast Implant Illness (BII)
• Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL), or significant fear of developing the disease

Are there alternative implant options for patients who no longer want implants?

Yes! Patients who had their implants placed cosmetically and still desire additional volume after removing their implants may benefit from fat grafting. Fat grafting is a technique that involves removing fat from one area of the body via liposuction, purifying the fat, then reinjecting it into the breast area. For patients who had their implants placed for breast reconstruction following a mastectomy, implants are not the only option to reconstruct a breast following breast cancer. Natural implant-alternative options include procedures like the DIEP flap which use the patient’s own skin and fat from the lower abdominal area to recreate the breast.
