Hairline placement
We all want the hairline we had when we were 20 years old. Unfortunately, it is when the hairline is too aggressively lowered in hair restoration that the results of a transplant will appear to be very unnatural. The hairline that you are happy with when you are 30 years old is not the hairline which would look natural as a 45 year old. As much as my patients may want me to be aggressive in lowering their hairlines, I counsel them to think about their hairline 10, 15 years later. As the existing hairline naturally recedes, the transplanted hairs will remain in place potentially creating an unnatural appearance. When designing the placement of a proper hairline, anticipated future loss needs to be considered to create the most natural results.
Hairline design
Hairlines are neither perfectly straight nor perfectly irregular. The design of the hairline is critical. Looking at the hairlines of men without hair loss, you will notice certain characteristics. Hairlines have subtle undulations or inconsistencies. Hairs grow in small groupings and do not grow in a staggered fashion. When transplanted follicles are placed in an entirely irregular manner, the results are unnatural once the hair from the transplanted follicles grow out.
Hair direction and angulation
Finally, one must pay meticulous attention to the direction and angle at which the hair follicles are transplanted. The most critical part of a hair transplant are the incisions the surgeon makes in the procedure. Each of these incisions determines the angle and the direction of hair growth. In order for the results to appear as undetectable as possible, the natural direction and angle of the existing hairline needs to be replicated in the newly transplanted hairline.
